    What Is a DOT Qualified SAP?

    A Substance Abuse Professional (SAP) is a credentialed clinician who evaluates employees who have violated a DOT drug and alcohol regulation. We assess the employee, recommend appropriate education or treatment, and conduct a follow-up evaluation — all in accordance with 49 CFR Part 40.

    Please note: The SAP does not determine employment status or guarantee return to duty. The SAP determines compliance with recommended education and/or treatment. The employer or DER manages return-to-duty testing and determines when safety-sensitive work may resume.

    Initial SAP Evaluation

    A face-to-face clinical evaluation to review the DOT violation and assess the employee's needs.

    Recommendations & Referral

    We provide recommendations for education, treatment, or both, with referrals to qualified providers when needed.

    Complete Recommendations

    The employee completes the recommended education or treatment. These costs are separate and paid directly to the provider.

    Follow-Up Evaluation

    After completion, a follow-up evaluation confirms compliance with the SAP's recommendations.

    Return-to-Duty & Testing Plan

    We provide required documentation and develop a follow-up testing plan. The employer or DER manages return-to-duty testing.

    The cost of any recommended education or treatment program is separate and determined by that provider. The $500 SAP fee covers the full clinical assessment, reporting, and follow-up process.

    When Is a SAP Evaluation Required?

    A SAP evaluation may be required when a DOT-regulated safety-sensitive employee has a DOT drug or alcohol program violation, such as a positive test or test refusal.

    This may apply to employees regulated by:

    FMCSA — Commercial drivers & CDL holders
    FAA — Aviation employees
    FTA — Public transit employees
    FRA — Railroad employees
    PHMSA — Pipeline employees
    USCG — U.S. Coast Guard personnel
