SAP Evaluations (DOT)

 

Substance Abuse Professional (SAP) evaluations are conducted in accordance with DOT regulations (49 CFR Part 40). Final cost may vary based on compliance requirements and recommended level of care.

Policies & Guidelines

Scheduling & Payment

Appointments must be scheduled prior to the session.
A valid card on file is required to secure your appointment.
Payment is processed at the time of your appointment.

Cancellation & Rescheduling Policy

A minimum of 24 hours’ notice is required to cancel or reschedule.
Appointments canceled with at least 24 hours’ notice may be rescheduled once at no cost.
Late cancellations and missed appointments will be charged in full.

Refund Policy

All services are non-refundable once completed.
No refunds are provided for missed appointments or late cancellations.
Partial refunds are not issued for services rendered.

Evaluation Services Disclaimer

Fitness for Duty and DOT SAP evaluations are professional assessment services and are not therapy.
Determinations are based on clinical judgment, available information, and applicable standards.
Payment does not guarantee any specific outcome or recommendation.
All evaluations and reports are conducted in accordance with applicable professional and regulatory guidelines.

Late Arrival Policy

Clients are expected to arrive on time for their scheduled appointment.

Arrivals more than 10–15 minutes late may require rescheduling
Late arrivals may result in a shortened session or forfeiture of the appointment

Confidentiality & Authorization

All services are confidential and handled in accordance with applicable privacy laws and professional standards.
Information may be disclosed with client authorization or as required by law.

*For DOT SAP and Fitness for Duty evaluations, results and required documentation may be shared with employers, Designated Employer Representatives (DERs), or other authorized parties as part of the evaluation process. *

Telehealth Policy

Services are provided via secure, HIPAA-compliant telehealth platforms, and clients are responsible for ensuring a private setting and reliable internet connection at the time of service. 

Report Turnaround

Reports are typically completed within 24–72 hours following the evaluation.

Additional Coordination

Additional communication or coordination with employers, legal representatives, or third parties beyond standard evaluation requirements may incur additional fees.

Expedited Services

Expedited report delivery may be available upon request for an additional fee.
