What is a SAP Evaluation?
At STIR, DOT Substance Abuse Professional (SAP) evaluations are conducted with both compliance and respect in mind. You’ll receive clear guidance through each step of the process while being treated as a person—not just a case.
A SAP evaluation is a federally required assessment for employees who have violated Department of Transportation (DOT) drug and alcohol regulations. The purpose of the evaluation is to determine what is needed for you to safely and compliantly return to duty.
As part of the evaluation, the SAP will assess your situation and determine:
- The appropriate level of care (education, treatment, or both)
- Clear recommendations for next steps
- Your readiness to return to duty (after completion of requirements)
- Whether follow-up testing will be required
The goal of this process is to support a safe, structured, and compliant return to work.
Services Include:
Initial Evaluation
A comprehensive, confidential assessment to understand your situation and determine next steps.
Treatment / Education Recommendations

Case Management & Progress Monitoring
Ongoing case management is provided throughout the SAP process to ensure a structured, efficient progression through all required steps.
Follow-Up Evaluation
Once all recommendations have been completed, a follow-up evaluation is conducted to assess compliance and determine eligibility for return to duty.

Coordination is provided with employers, Designated Employer Representatives (DERs), and Medical Review Officers (MROs) to help streamline the process, prevent miscommunication, and ensure all steps meet DOT requirements.
